從經典例文中學習英文論文寫作（第1期：Semaglutide）

大家好，我是醫(yī)藥行業(yè)的一名從業(yè)人員，業(yè)余時間會從事一些專業(yè)翻譯方面的工作，斷斷續(xù)續(xù)有幾年時間了。 其實翻譯并不神秘，最重要之處無非是多練多看，從好的文章中提取最地道的表達，融入到自己的表達之中。因此，我總是會抽出很多時間進行一些翻譯的練習來提升自己。 在做練習的時，我有一天想到，既然已經做了一些工作，那么為何不同時把這些工作內容分享出來呢。一方面可以幫助有需求的小伙伴自我提升，一方面收到別人點評和提問對自己也是一種收獲。（在這里感謝陳數(shù)羊前輩給我的帶來的啟發(fā)。另外，本篇文章中的很多內容也來自數(shù)羊老師的教學分享）。 我做練習的方式是非�！氨俊钡�，會先把文章翻譯成中文，再自己翻譯回去對照原文進行學習。本文的回翻統(tǒng)一用了機器翻譯，雖然在自己做練習時不建議這樣做。另外想說一下，在大家寫作過程中還是非常建議能以機器翻譯結果作為參考的，因為現(xiàn)在的機器翻譯水平已經比較穩(wěn)定和優(yōu)秀了，站在它的肩膀上，對論文寫作有很大幫助。那么我們需要學習的其實是，如何在機器翻譯上進一步的提高自己。 今天要分享的一篇文章節(jié)選自司美格魯肽的臨床試驗論文：Two-year effects of semaglutide in adults with overweight or obesity: the STEP 5 trial。 司美格魯肽（semaglutide）是丹麥諾和諾德公司（Novo Nordisk）這兩年大火的產品，主要作用是促進胰島素分泌。胰島素可以降低血糖，用于治療糖尿病，也可以用于減肥。 關于semaglutide的臨床試驗是一系列的，目前已經有8個關于semaglutide的臨床試驗結束且發(fā)布了結果，相關文章基本都發(fā)表在醫(yī)學頂刊上，我們要討論的這篇來自Nature Medicine。 那后面就讓我們來看一看文章中的這些地道表達吧！ （您的支持和鼓勵是我前進的動力。如果喜歡的朋友們多，我后續(xù)會持續(xù)更新不同的內容。您如果有比較好的想要一起閱讀學習的文章，也可以告訴我，我們一起學習進步。由于篇幅原因，最好是試驗類文章不要是綜述哦。） |

"incorporating" 強調整合或合并，使某物成為整體的一部分，有納入的意思，而 "combined with" 更多地描述兩個或多個元素相互結合，"including" 更多地表示列舉或簡單地添加某些元素，而不一定要與其他部分緊密結合。這幾個詞都可以翻譯為包括，結合。如果是出現(xiàn)在醫(yī)學博士考試中，那大概率會在選擇題中選擇一個替換詞，比如include。兩者是近義詞，不過incorporate有更明顯的“納入”意思，而include就只是“包含”。比如在這句話中，行為干預“納入”了飲食和活動成為一個整體，在具體表達時可以直接翻譯為“包含/包括”。 2. "modifications in" 往往描述微小的調整，而 "change" 是一個更為普遍的術語，用于描述從微小到顯著的任何變化或轉變。可以互相替換。 3. "remains"和"is still"這兩個短語都強調某種狀態(tài)或情況的持續(xù)性。但"remains" 有時給人一種稍微正式的感覺，而 "is still" 在日常對話中可能更為常見�？梢曰ハ嗵鎿Q。 4. "foundation" 通常強調某事的核心原則，有基石的含義，而 "basis" 則強調邏輯、原因或證據(jù)。也可以互相替換。 然而，由于行為干預通常與臨床上有意義和可持續(xù)的體重減輕無關，因此建議將藥物治療作為體重指數(shù)（BMI）至少為 30 kg m-2 或患有體重相關共病癥至少為 27 kg m-2 的人群長期體重管理的額外手段。 原文：However, because behavioral intervention is often not associated with clinically meaningful and sustainable weight loss, pharmacotherapy is recommended as an additional tool for long-term weight management in people with a body mass index (BMI) of at least 30 kg m–2, or at least 27 kg m–2 in those with weight-related comorbidities. 譯文：However, since behavioral interventions are usually not associated with clinically meaningful and sustainable weight loss, it is recommended that drug therapy be used as an additional tool for long-term weight management of people with a body mass index (BMI) of at least 30 kgm-2 or with weight related comorbidities of at least 27 kgm-2. 1. "because" 更直接，而 "since" 則有一種基于已知或公認事實的含義。兩者在此可以互換。 2. 另外學習一下“it is recommended that … be used as …” 可以寫成 “… is recommended as …” 3. 有統(tǒng)計學意義的“意義”用 significant/significance 基本是約定俗成的用法，但有臨床意義的“意義”英語用詞還不那么統(tǒng)一，可以見到用 significant/significance、meaningful/meaningfulness、important/importance 等。同時，clinically/clinical 還可用 practically/practical 或者 medically/medical 等。 4. 藥物療法/治療一般可能比較想到的表達是drug treatment/therapy，也可以用pharmacotherapy替換。在表示“建議”的時候，一般我們會想到advice和suggest，但很多時候我們也可以用recommend推薦替換，而且從說話姿態(tài)上更平等一些。 5. as an additional tool，這個表達也記一下，中文常遇到“作為……補充/其他XX方法/手段”，我們當然可以用as a complementary/supplementary XX method/approach，也可以用additional tool。 Semaglutide 是一種胰高血糖素樣肽-1（GLP-1）類似物，被批準用于治療二型糖尿�。ǹ诜� Semaglutide 和皮下注射 Semaglutide）和降低二型糖尿病和心血管疾病患者的心血管事件風險（僅皮下注射 Semaglutide）。 原文：Semaglutide is a glucagon-like peptide-1 (GLP-1) analog approved for the treatment of type 2 diabetes (oral semaglutide and subcutaneous semaglutide) and for reducing the risk of cardiovascular events in people with type 2 diabetes and cardiovascular disease (subcutaneous semaglutide only). 譯文：Semaglutide is a glucagon like peptide-1 (GLP-1) analog, which is approved for the treatment of type 2 diabetes mellitus (oral semaglutide and subcutaneous semaglutide) and reducing the risk of cardiovascular events in patients with type 2 diabetes mellitus and cardiovascular disease (subcutaneous semaglutide only). 1. 這段翻譯的非常好，只是 approved for 如省略 which is 可略顯簡潔。醫(yī)學類論文/產品研究中經常出現(xiàn)FDA approved和FDA cleared，它們兩個有什么區(qū)別呢？approved比cleared要求更高，比如在藥物上，必須要證明安全性和有效性，才可以approved，approved后的產品也意味著獲益比已知風險大。而cleared一般適用于跟現(xiàn)有產品相似的新產品，而不一定是通過了獨立的安全性試驗。說到這里，有時候可能還會看到FDA registered和FDA certificated，這兩個表達跟FDA的認可沒有什么關系，如果某個產品用了這些表達宣傳，那大概率就是生產商要騙人了。 根據(jù) semaglutide 對肥胖患者的治療效果（STEP）臨床試驗計劃的結果，美國、歐洲、英國和加拿大批準皮下注射 semaglutide，劑量為2.4 mg，每周一次，用于超重（BMI≥27 kg m-2，至少有一種體重相關共病癥）或肥胖（BMI≥30 kg m-2）的成人的體重管理。 原文：At a dose of 2.4 mg once-weekly, subcutaneous semaglutide was approved in the United States, Europe, the United Kingdom and Canada for weight management in adults with overweight (BMI ≥ 27 kg m–2 with at least one weight-related comorbidity) or obesity (BMI ≥ 30 kg m–2), based on results from the Semaglutide Treatment Effect in People with Obesity (STEP) clinical trial program. 譯文：According to the results of the treatment effect of semaglutide on obese patients (step) clinical trial plan, the United States, Europe, the United Kingdom and Canada approved the subcutaneous injection of semaglutide at a dose of 2.4 mg once a week for the weight management of overweight (BMI ≥ 27 kgm-2, at least one weight related comorbidity) or obese (BMI ≥ 30 kgm-2) adults. 1. 這句話用詞都沒有什么問題，但是犯了機器翻譯和不少水平不錯譯者翻譯中最常見的毛病，那就是介詞短語比較多的時候排序習慣還是按照中文的來，相對重要的部分放的太靠后了，因此翻譯下來帶有濃濃的中式英語風格，另外主要結構也沒有用更符合英語表達習慣的被動語態(tài)。這句話有八個介詞短語，At a dose of 2.4 mg once-weekly，in the United States…，in adults，with overweight，with at least one weight-related comorbidity，or obesity，based on results，from the Semaglutide Treatment Effect in People with Obesity (STEP) clinical trial program，一定要多多斟酌，熟悉其排序的英語表達習慣。 2. management也可以多關注一下，有時候可以替換control，比如血壓控制 blood pressure control，也可以換詞blood pressure management。 在沒有 2 型糖尿病的參與者的 STEP 1 和 3 試驗中，使用 semaglutide 2.4 mg 觀察到平均減去安慰劑的體重減輕在 68 周分別為 12.4% 和 10.3% 。 原文：In the STEP 1 and 3 trials in participants without type 2 diabetes, average placebo-subtracted weight losses of 12.4% and 10.3%, respectively, were seen with semaglutide 2.4 mg at week 68. 譯文：In the STEP 1 and 3 trials of participants without type 2 diabetes, using semaglutide 2.4 mg, we observed that the average weight loss after subtracting placebo was 12.4% and 10.3% at 68 weeks, respectively. 1. 這句話又是一個很好的范例，表達上雖然沒什么問題，但是表達帶有濃重的中文習慣。注意，用被動語態(tài)把要表達的重點提前！ 2. "seen"主要與簡單的視覺感知有關，而 "observed" 除了表示看到外，還帶有更深入、仔細或專業(yè)的觀察的含義。這句話參考譯文的用詞表現(xiàn)甚至更好一些。 STEP 試驗項目中先前的研究僅限于長達 68 周的治療持續(xù)時間。 原文：Previous studies in the STEP trial program have been limited to treatment durations of up to 68 weeks. 譯文：Previous studies in the STEP trial were limited to treatment duration of up to 68 weeks. 1. 這句話有兩個關注點，一個是時態(tài)，一個是單復數(shù)。如果用現(xiàn)在完成時，表示 STEP 研究還在繼續(xù)，沒有結束。用過去式表示 STEP 研究已經完成。這里要注意區(qū)分一下到底需要用哪種時態(tài)表達。另外文中用了 up to 意思為最高達到，不同試驗的 treatment duration 是不同的，這里應該用復數(shù)。 本文報道的 STEP 5 研究為期 2 年，旨在評估每周一次皮下注射 2.4 mg semaglutide 與安慰劑相比，作為行為干預的輔助手段，針對肥胖（BMI ≥ 30 kg/m² ）成年人，或超重（BMI ≥ 27 kg/m² ）成年人，且至少存在一種與體重相關的共病癥且沒有糖尿病，對體重和心血管代謝風險因素的長期影響。 原文：The 2-year STEP 5 study reported herein was conducted to evaluate the long-term effect of once-weekly subcutaneous semaglutide 2.4 mg compared with placebo, as an adjunct to behavioral intervention, on body weight and cardiometabolic risk factors, in adults with obesity (BMI ≥ 30 kg m–2), or with overweight (BMI ≥ 27 kg m–2) and at least one weight-related comorbidity, without diabetes . 譯文：The 2-year step 5 study reported in this paper aims to evaluate the long-term impact of weekly subcutaneous injection of 2.4 mg semaglutide on weight and cardiovascular metabolic risk factors in obese (BMI ≥ 30 kg/M ²  or overweight (BMI ≥ 27 kg/m ² ) adults with at least one weight related comorbidity and no diabetes compared with placebo.1. 學習一下 herein 的使用，可以代替 here，in this paper，in this study 等表達。原文在這段話的處理上更簡潔，是我們需要學習的地方。 2. "was conducted to" 描述了已經發(fā)生的事情的目的，而 "aims to" 描述了現(xiàn)在或未來的計劃或活動的目標。過去時態(tài)的被動語態(tài)更符合文章意思和英語習慣。 3. Cardiometabolic 與 cardiovascular metabolic，經過查證，后者更常見一些，但前者是一個專有名詞。因此在做專業(yè)詞匯查證的時候需要仔細。 4. "impact" 往往帶有更強烈或更直接的影響的含義，而 "effect" 更為中性，指的是某個原因的直接結果或后果。在這里的影響用 effect 更好些，可以與標題呼應。 5. Without 和 and no 都可以用在這里。 6. 這句話有十個介詞短語。譯文的表現(xiàn)可以說相當不錯，只需調整compared with placebo 的位置，另外做一些表達上的小微調。 這項3期、隨機、雙盲、安慰劑對照、多國試驗代表了迄今為止使用 semaglutide 進行體重管理的最長研究。 原文：This phase 3, randomized, double-blind, placebo-controlled, multinational trial represents the longest study of the use of semaglutide for weight management to date. 譯文：This phase 3, randomized, double-blind, placebo-controlled, multinational trial represents the longest study of weight management using semaglutide to date. 1. 這句話比較簡單，譯文的完成度很高。to date學習一下，“截止到目前”。 2. 這句話只有一點小瑕疵，就是要把重點“使用 semaglutide 進行”放在前邊。這樣就和原文完全一致了。 共同主要終點是從基線到第 104 周體重的百分比變化，以及在第 104 周體重減輕程度至少為基線體重的 5%。 原文：Co-primary endpoints were percentage change in body weight from baseline to week 104 and achievement of weight loss of at least 5% of baseline weight at week 104. 譯文：The common primary endpoint was the percentage change in body weight from baseline to week 104 and a weight loss of at least 5% of baseline body weight at week 104. 1. 譯文在這里犯了一個比較大的錯誤，共同主要終點 Co-primary endpoints 翻譯成了共識中的主要終點 common primary endpoint。所以專業(yè)詞匯最好反復校對。 2. 譯文單復數(shù)用錯了。由于中文習慣不區(qū)分單復數(shù)，因此譯文中容易出錯，需要小心。 3. weight loss 前加入 achievement 可以使表達更地道，呼應percentage change in body weight。在英文表達時候要學會加上這些實際含義不大但卻影響整體觀感的詞匯。 |
